Upon treatment completion, clients will engage in a re-assessment process with the social worker and nurse practitioner. Your primary care provider will also receive the results of the assessment. Clients will also receive a 3 month post-treatment follow up to discuss relevant concerns with social worker.
During this follow up, recommendations can be made for further treatment, such as stepping down to the outpatient program at BANA or the weCHC to engage in the additional services that each program may offer or you may be provided with referrals to other community organizations.
